In most cases of male infertility, the diagnosis and treatment will likely involve removing sperm from the reproductive tract to use with In Vitro Fertilization (IVF) and/or Intra-Cytoplasmic Sperm Injection (ICSI).

Those men with hypothalamic or pituitary disorders that create hormone imbalances that are amenable to treatment should be treated with the appropriate hormone replacement therapy for their specific condition.

Some azoospermic patients with very low testosterone might also benefit from hormone replacement as it might increase sperm production enough to find some sperm in the ejaculate or make it easier to find sperm surgically.

Testosterone should NEVER be given as hormone replacement to men wishing to father children. It actually has the opposite effect on fertility and causes sterilization.

Other medications that boost testosterone production and don’t harm sperm production can be used safely. These may include Human Chorionic Gonadotropin (HCG), Follicle Stimulating Hormone (FSH), Clomid, and Arimidex and they might be used alone or in combination to improve a man’s hormonal balance depending on the individual circumstance.

It is important to closely monitor patients on these medications to insure the proper dosage of the medication and to reduce or avoid potential side effects. Typically, a patient will need to be on these medicines for three-18 months to see results, again depending on the individual’s specific condition.
