{"id":3233,"date":"2026-10-02T06:50:39","date_gmt":"2026-10-02T06:50:39","guid":{"rendered":"https:\/\/www.malereproduction.com\/blog\/?p=3233"},"modified":"2026-10-02T06:56:34","modified_gmt":"2026-10-02T06:56:34","slug":"why-patients-travel-from-around-the-world-for-male-fertility-treatment-and-vasectomy-reversal","status":"publish","type":"post","link":"https:\/\/www.malereproduction.com\/blog\/why-patients-travel-from-around-the-world-for-male-fertility-treatment-and-vasectomy-reversal\/","title":{"rendered":"Why Patients Travel From Around the World for Male Fertility Treatment and Vasectomy Reversal"},"content":{"rendered":"\n<p><strong><em>By <a href=\"\/about-cmrm\/dr-philip-werthman\/\">Philip Werthman, MD, MMH<\/a><\/em><\/strong><\/p>\n<p>Patients regularly travel from across the United States&mdash;and from around the world&mdash;to Los Angeles for <a href=\"\/vasectomy-reversal\/\">vasectomy reversal<\/a> and <a href=\"\/male-infertility\/\">male fertility treatment<\/a>.<\/p>\n<p>Why travel thousands of miles for a procedure that may technically be available closer to home?<\/p>\n<p>Because when surgery is highly specialized, experience matters.<\/p>\n<p>For more than 35 years, male reproductive microsurgery has been the focus of my professional life. I have performed thousands upon thousands of microsurgical procedures, helping thousands of families achieve pregnancies and have children.<\/p>\n<p>Microsurgery is not simply one part of my practice.<\/p>\n<p><strong>It is the focus of my practice.<\/strong><\/p>\n<h1>Experience Cannot Be Compressed<\/h1>\n<p>There are many talented young physicians entering reproductive urology today. They are well trained and represent the future of the specialty.<\/p>\n<p>But there is one thing that cannot be taught during fellowship or compressed into a few years of practice: enormous surgical case volume accumulated over decades.<\/p>\n<p>The fertility landscape has changed. <a href=\"\/procedures\/ivf-icsi\/\">IVF<\/a> has become increasingly common, more physicians are competing for the same surgical patients, and cases are distributed among many different practices. As a result, it is difficult for a young surgeon today to accumulate the volume of vasectomy reversals, vasoepididymostomies, microTESE procedures and other complex reproductive microsurgeries that surgeons in a highly specialized practice could accumulate over decades.<\/p>\n<p>That experience matters.<\/p>\n<p>Microsurgery is learned through repetition&mdash;not simply hundreds of repetitions, but thousands. You encounter straightforward anatomy, abnormal anatomy, scar tissue, previous failed operations, unexpected obstruction and situations that cannot be recreated in a textbook.<\/p>\n<p>Thirty-five years of those experiences influences every decision I make in the operating room.<\/p>\n<h1>Surgery Should Be Efficient, Not Rushed<\/h1>\n<p>Patients and other physicians sometimes notice that I operate quickly.<\/p>\n<p>That is not because I am rushing.<\/p>\n<p>It is because I have performed these procedures so many times that unnecessary movements have gradually disappeared.<\/p>\n<p>I think of a highly refined operation almost like a choreographed ballet. Every movement has a purpose. The instruments are where they need to be. Every step flows into the next. There is very little wasted motion or unnecessary dissection.<\/p>\n<p>That efficiency is the product of repetition.<\/p>\n<p>The objective is not to finish an operation quickly. The objective is to perform it precisely and efficiently while minimizing unnecessary tissue manipulation.<\/p>\n<h1>Developing Better Ways to Perform These Operations<\/h1>\n<p>I have never been satisfied simply performing procedures the way they have always been performed. Throughout my career, I have continually asked whether we can make them less invasive and easier for patients while maintaining the microsurgical precision required for success.<\/p>\n<p>That philosophy led me to develop the mini-incision microsurgical vasoepididymostomy.<\/p>\n<p>Vasoepididymostomy is one of the most technically demanding operations in reproductive microsurgery. It may be required during a vasectomy reversal when a secondary obstruction has developed within the epididymis. Instead of simply reconnecting the vas deferens, the surgeon must create a microscopic connection between the vas and an individual epididymal tubule.<\/p>\n<p>My mini-incision approach was designed to accomplish that reconstruction through a less invasive operation, with the goal of reducing tissue disruption, postoperative discomfort and recovery.<\/p>\n<p>I applied the same philosophy to mini-incision microsurgical testicular sperm extraction, or microTESE, for men with severe sperm-production problems and nonobstructive azoospermia.<\/p>\n<p>In these patients, sperm production may exist only in tiny isolated areas within the testicle. The challenge is not simply finding sperm. It is finding those areas while minimizing unnecessary injury to the testicle and preserving its blood supply and function.<\/p>\n<h1>Difficult Cases Are Where Experience May Matter Most<\/h1>\n<p>Many patients who travel to see me are not straightforward cases.<\/p>\n<p>Some have undergone unsuccessful surgery elsewhere. Some have very long obstructive intervals after vasectomy. Some require vasoepididymostomy rather than a standard vasovasostomy. Others have severe abnormalities of sperm production, previous testicular surgery, genetic conditions or extremely challenging sperm retrievals.<\/p>\n<p>Some have been told that there is little or no hope of having a biological child.<\/p>\n<p>These are often the cases I find most meaningful.<\/p>\n<p>After thousands of procedures, I have encountered an enormous range of surgical situations. That experience allows me to recognize problems quickly, adapt during an operation and choose an approach based upon what I actually find rather than simply what I expected to find.<\/p>\n<h1>A Concierge-Level Private Practice<\/h1>\n<p>There is also another reason patients travel to us: the experience is different from navigating a large hospital or university system.<\/p>\n<p>We are a concierge-level private practice.<\/p>\n<p>Patients are not passed through layers of trainees or a complicated institutional structure. We do not have medical students or residents performing portions of these highly specialized operations on our patients.<\/p>\n<p>When someone travels across the country or across the world to see me, they are traveling to be treated by me.<\/p>\n<p>That is important.<\/p>\n<p>Fertility treatment is already stressful. Traveling for surgery adds another layer of complexity. Our goal is to remove as much of that complexity as possible.<\/p>\n<p>We try to streamline the entire experience&mdash;from consultation and scheduling through surgery, postoperative care and follow-up&mdash;so that patients are not spending their time navigating departments, telephone trees and multiple layers of administration.<\/p>\n<p>Patients should feel that they can reach us, understand what is happening and know who is responsible for their care.<\/p>\n<p>This is highly personal medicine, and we try to practice it that way.<\/p>\n<h1>Recognition Is Meaningful&mdash;But Results Matter More<\/h1>\n<p>Over the course of my career, I have been fortunate to receive many of the honors available in my field, including recognition on numerous Top Doctor lists for many consecutive years and the American Fertility Association&#8217;s Illumination Award.<\/p>\n<p>I am grateful for those honors.<\/p>\n<p>But the recognition that matters most comes later.<\/p>\n<p>Sometimes it is an ultrasound image sent after a long-awaited pregnancy. More often, a year or two after surgery, it is a photograph of a baby. Occasionally a family comes back years later and brings their child into the office so I can meet them.<\/p>\n<p>Those are the moments that remind me why I chose this field.<\/p>\n<h1>Why Travel for Fertility Surgery?<\/h1>\n<p>Many of these procedures can technically be performed elsewhere.<\/p>\n<p>But highly specialized microsurgery is profoundly dependent upon experience, judgment and technique.<\/p>\n<p>If you are considering an operation that you hope to undergo only once, I believe it is reasonable to look beyond geography and ask a different question:<\/p>\n<p><strong>Who has devoted their career to doing this?<\/strong><\/p>\n<p>For me, male reproductive microsurgery has never simply been a procedure I perform.<\/p>\n<p>It has been my life&#8217;s work&mdash;35 years spent performing these operations, refining them, developing new techniques and taking care of some of the most challenging patients in male infertility.<\/p>\n<p>When someone travels hundreds or thousands of miles and places that trust in me, I take that responsibility very seriously.<\/p>\n<p>Because ultimately, the goal is not simply a successful operation.<\/p>\n<p><strong>The goal is helping people create the family they dreamed of having.<\/strong><\/p>\n\n","protected":false},"excerpt":{"rendered":"<p>By Philip Werthman, MD, MMH Patients regularly travel from across the United States&mdash;and from around the world&mdash;to Los Angeles for vasectomy reversal and male fertility treatment.
